Round-Up Online

Please be sure you have your Unique Family ID Number ready. This was included in the Round-up mailing that you received in early July.

The Round-up Online process is divided into Part One: Online Survey, Part Two: Downloadable Forms and Part Three: Optional but Important. We suggest having an address or phone book accessible, and please be sure you have your Unique Family ID Number available.

Please read all instructions below.

PART ONE: Round-Up Online Survey
Part One is presented in a survey format. You must enter your Family ID number that was mailed to you in July. You need to complete Part One in its entirety once you have begun. It cannot be saved. You must answer every question marked with a red asterisk (*). If you choose, you may print each completed page for your records.

You can go back to previous pages in the survey and update existing responses until the survey is finished or until you have exited the survey. After the survey is finished, you will not be able to re-enter the survey. Please make sure to proofread everything before you click "submit." You are not able go back in to make changes.

PART TWO: Round-Up Online *REQUIRED* Downloadable Forms
There are some forms required by each family that must be maintained as a hard copy. Please print and complete each required form. Forms requiring signatures must contain all signatures on the same form. In some cases, one form, per child, is required. You must bring the completed forms with you to Round-Up.

Athletics and Activities Forms
Forms are required for partipation in athletics/extracurricular activities (i.e. sports, speech, debate, drama, etc).

MSHSL 2009-10 Athletic Eligibility Statement Required to participate in athletics and/or extracurricular activities, Grades 7-12
Physical Required every three years, signed and dated by physician

Health Services Forms

Annual Health Information Form Required every year for every student enrolled
Emergency Medication or Management Plan As needed
Administration of PRESCRIPTION Medication As needed
Administration of NON-PRESCRIPTION Medication As needed

Transportation
A complicated Minnesota state law guarantees equal school bus transportation to all students, attending either a public or a private school, or financial aid as a substitute. The process for each family to receive this financial aid is dependent upon which public school district the family resides in. Please review the form listed below; a listing of school districts and their requirements is provided. Then complete Form 1000, as noted, and bring to the Red Knight Round-up Transportation Table.

Laptop
A laptop (provided by BSM) is required for any student enrolled in either grade 7 or 8. An agreement form must be submitted to the junior high prior to release of the laptop to the student. One agreement page, containing all signatures, is preferred. If a student resides at more than one residence, additional agreement forms may be submitted on behalf of the student.

PART THREE: Optional but Important
Order a Senior High Yearbook Yearbooks can only be ordered online and with a credit card. Junior High yearbooks will be ordered during the school year.
